•2011-2012 Belmont Redwood Shores School District Notice to Parents

(The following 3 forms, which are located at the end of the above District Notice,  must be signed and returned to the teacher each school year. I also have attached them below.)

•Technology Use Agreement form

•Parent Photo Release form

•Parental Receipt Agreement of District Policies Handbook 

The volunteer form must be filled out each year and is due by Sep. 2, 2011; your current negative TB test must be on file in the office; and you must be cleared by the  Dept. of Justice (Megan's Law) BEFORE you can volunteer in the classroom.

•Volunteer Form (due Sep 2, 2011)

•1st Period Milk Order Form (Sep-Jan) (Due by Sep. 2, 2011)

•Free/Reduced Lunch/Milk form (for those families who qualify under the income eligibility guidelines.)

•Driver's Insurance/Information Form (both forms need to be filled out and turned in to your child's teacher with a copy of your auto insurance, each year, if you intend to drive on any fieldtrips this year.)

•Report of Health Exam for School Entry (Required for incoming 1st graders)

•Health Hold Harmless Form  (Please note that no medication (Rx or OTC)  can be used by students without a Hold Harmless Form signed by your pediatrician.  This includes asthma inhalers, chap stick, sunscreen, cortisone cream, cough drops/lozenges, etc.  Teachers will confiscate all of these types of products and turn them in to the office until a signed form is received.)
